We discussed a case of PCNSL in an HIV patient and reviewed the literaure about the pathogenic role of EBV. In such immunocomprisised patients, the detection of EBV transcript in all of the lymphoma cells is indicative of a pathogenetic role of the virus and recent NGS resutls indicate that these lymphomas have a distinct signature from EBV negative ones related to the tolerfogeni microenviroment
