Recently a new DRB1*1454 allele was described by Blasczyket al.
DRB1*1454 differs from DRB1*1401 in one nucleotideat position 51 of exon
3 resulting in a Tyr/His exchangeat position 112 of the HLA molecule.
From this study, theyconclude that DRB1*1454 is most likely the most
commonDRB1*14 allele and is therefore of potential clinical relevancefor
donor selection in stem cell transplantation. To establish theimportance
of this new allele in our hematopoietic stem celltransplant patients, we
retyped 34 recipient-donor pairs withDRB1*1401. DRB1*1454 was present in
30 recipients (88%)and in 28 of the 32 donors/cord blood units (88%).
For 2 cordblood units, not enough DNA was present for re-typing. In
26pairs, both recipient and donor carried DRB1*1454 (81%), in 2pairs
DRB1*1401 was present in recipient and donor (6%) andin 4 pairs an
incompatibility for the DRB1*14 allele was found(13%). DRB1*1401 and
1454 are found in association withDRB3*0201 and 0202 respectively. The
MLCs performedbetween the DRB1*14 allele mismatched combinations
werenegative or weakly positive (mean RR¼5%, range 0–14%). Inconclusion,
the majority of the previously DRB1*1401 typedindividuals are DRB1*1454.
The amino acid (Tyr vs. His)differences at position 112 of the DRB1*14
allele and the DRB3difference do not result in a primary MLC response.
The po-tential clinical relevance of the difference between DRB1*1401and
1454 allele in stem cell transplantation remains to bedetermined but is
likely to be of minor importance due to theabsence of cellular
recognition
